The Form Input module enables you to create a form for your end-users to respond to different kinds of interactions on IA engagements in one screen. For instance, you can send a form to an end-user where they enter their details and answer multiple-choice questions all at once without moving to the next screen.
This can be seamlessly done using the Form Input module. The Form Input module enables you to add different kinds of modules within it, including the following:
Open Response
Message
Multiple Choice
File Upload
Location modules
Using the Form Input Module
To use the Form Input module, complete the following:
Open the Ushur if you want to add a Form Input module.
Click the following Form Input icon. This displays the Form Input dialog box.
In the Label box, enter a unique name for the Form Input module within Ushur.
In the next box, click the Plus icon to see the different options available.
This displays the different options available to add to a form.Note
File Upload module supports JPG or JPEG, PNG, PDF, XLS, XLSX, TXT, CSV, DOC, and DOCX file types.
Select the option you want to add to the form.
Note
You must configure the selected module as a stand-alone module within the Ushur builder. This includes names, assigning variables, and all the mandatory fields within this module.
Configuring to Dynamically Display Options based on User Response
Ushur provides you the ability to configure additional displays of choices based on user response. You can link any of the four modules (except the Message module) within the Form Input module to provide users with options to view specific responses dynamically, based on their selection.
Sample Scenario
As an enterprise, you may want to first let your users know of two options - Gold Membership and Silver Membership. Based on the user selection, you may want to provide more information about the type of membership selected. This enables the user to dynamically view information based on the response they choose. In such scenarios, you can use the Form module to configure two modules and link them to allow users to select and view options dynamically. For this scenario, the option can be configured using the Open Response module and the Multiple Choice module.

You can configure the sub-modules using a combination/all /multiple of the four modules within the Form Input module as a part of this process. For example, you can link the Multiple Choice module, which on user input will open the File Upload module.
To add another module within the form, click the Plus icon select Open Response, and specify the details.
Select the Toggle Response check box and the Refresh on Change check box. This will ensure that any change in the module will be reflected and displayed to the user on a refresh.
Click the plus icon, select, and add the Multiple Choice module.
Specify the details and select the Refresh on change option.
Select the Display Condition, and ensure that the Ushur variable that accepts the User response is set to the value Yes. If you do not do this, the module will not function as expected.
Click Save to add the form to the Ushur.
